Underwood Tax & Bookkeeping Service is proud to offer the completion of Form 1096 for you and 1099s for your payees; as well as Form W3 and W2s for any Employee

For Contractors we must receive the first page of Form W-9, for each individual or entity you are reporting on, as well as the amount paid.

For Employees we must receive Form W-4 for each employee you are reporting on as well as the amount paid.

Forms W-4 and W-9 need only be completed the first year we create these documents or any time there is a change of information. For subsequent years we only need the information about the amounts of money as it pertains to each individual.
